How To Register
We register all new patients with our doctors on a rotational basis. However, you are free to express a preference for a particular doctor and this will be noted on your computer records.
All new patients are required to complete a national registration form before they can be seen/ treated at the surgery. You will be asked to provide evidence of your identity in the form of a passport or photo driving licence, as well as proof of your address, eg a recent utility bill. We ask all new patients to complete a simple questionnaire about their health.
New Patient Medical Checks
As a newly registered patient, we would like you to make an appointment with one of our health care assistants for a basic health check. At this appointment your height, weight, blood pressure and urine will be checked. Please ensure that all family members over five years of age attend. It will take approximately 15 minutes per person.
Please bring your completed questionnaire to the health check and also a specimen of your urine (a container is available at reception). This basic medical information is important to your new doctor. Until we receive your medical records from your last doctor it is essential to have your help with this information to ensure we can offer you the best possible care.
